Doctor's Order:
Start a Vasopressin CRI at 0.5 mU/kg/min
Patient Weight:
10 kg
Standard Dilution Protocol (Varies By Hospital):
Dilute Vasopressin with 0.9% NaCl where 1 mU/kg/min = 1 ml/hr and create a 12 hour CRI at that rate. Make sure the final diluted concentration of Vasopressin is below 0.1 1 unit/ml. If it's not, dilute it further
Vasopressin Concentration:
20 U/ml
Convert the Vasopressin from U to mU:
(20 U/ml) x (1,000 mU/U) = 20,000 mU/ml concentration of Vasopressin
Complete the Math:
(1 mU/kg/min) x (10 kg) = 10 mU/min of Vasopressin
(10 mU/min) x (60 min/hr) = 600 mU/hr of Vasopressin
(600 mU/hr) / (20,000 mU/ml) = 0.03 ml/hr of Vasopressin
(0.03 ml/hr) x (12 hr) = 0.36 ml Vasopressin
(12 ml Total Fluid Volume) - (0.36 ml Vasopressin) = 11.64 ml 0.9% NaCl
Making Sure the Final Concentration is Under 1 unit/ml:
(0.36 ml Vasopressin) x (20 U/ml) = 7.2 U of Vasopressin
(7.2 U) / (12 ml Total Fluid Volume) = 0.6 U/ml Vasopressin
The Result:
You will need 0.36 ml of Vasopressin to mix with 11.64 ml of 0.9% NaCl. This will result in 1 mU/kg/min = 1 ml/hr. To run the Vasopressin CRI at 0.5 mU/kg/min, set the fluid rate of your CRI to 0.5 ml/hr
*Note Per Plumb's as of 01/16/23: Vasopressin "is incompatible with ampicillin, diazepam, furosemide, insulin (regular), and phenytoin."
