Doctor's Order:
Start a Metoclopramide CRI at 2 mg/kg/day
Patient Weight:
30 kg
Standard Dilution Protocol (Varies By Hospital):
Dilute Metoclopramide with 0.9% NaCl where 1 mg/kg/day = 1 ml/hr and create a 24 hour CRI at that rate
Metoclopramide Concentration:
5 mg/ml
Complete the Math:
(1 mg/kg/day) x (30 kg) = 30 mg/day of Metoclopramide
(30 mg/day) / (24 hr/day) = 1.25 mg/hr of Metoclopramide
(1.25 mg/hr) / (5 mg/ml) = 0.25 ml/hr of Metoclopramide
(0.25 ml/hr) x (24 hr) = 6 ml Metoclopramide
(24 ml Total Fluid Volume) - (6 ml Metoclopramide) = 18 ml 0.9% NaCl
The Result:
You will need 6 ml of Metoclopramide to mix with 18 ml of 0.9% NaCl. This will result in 1 mg/kg/day = 1 ml/hr. To run the Metoclopramide CRI at 2 mg/kg/day, increase the fluid rate of your CRI to 2 ml/hr
*Note Per Plumb's as of 10/11/21: "Metoclopramide is reported to be physically incompatible when mixed with the following drugs: ampicillin sodium, calcium gluconate, chloramphenicol sodium succinate, cisplatin, erythromycin lactobionate, methotrexate sodium, penicillin G potassium, sodium bicarbonate, and tetracycline."
