Doctor's Order:
Start a Heparin CRI at 20 units/kg/hr. Draw up 10 hours worth of Heparin
Patient Weight:
6 kg
Heparin Concentration:
1,000 units/ml
Complete the Math:
(20 units/kg/hr) x (6 kg) = 120 units/hr of Heparin
(120 units/hr) / (1,000 units/ml) = 0.12 ml/hr of Heparin
(0.12 ml/hr) x (10 hr) = 1.2 ml Heparin
The Result:
You will need 1.2 ml of Heparin to run the CRI at 20 units/kg/hr or 0.12 ml/hr for 10 hours
*Note Per Plumb's as of 10/11/21: "Heparin sodium is reported physically incompatible when mixed with the following solutions or drugs: sodium lactate 1/6 M, amikacin sulfate, chlorpromazine HCl, ciprofloxacin, codeine phosphate, cytarabine, daunorubicin HCl, diazepam, dolasetron HCl, doxorubicin HCl, doxycycline hyclate, erythromycin lactobionate, gentamicin sulfate, hyaluronidase, hydroxyzine HCl, hydromorphone HCl, ketamine HCl, levorphanol bitartrate, meperidine HCl, methadone HCl, mitoxantrone, morphine sulfate, pentazocine lactate, phenytoin sodium, polymyxin B sulfate, and vancomycin HCl."
