Doctor's Order:
Start a Famotidine CRI at 8 mg/kg/day
Patient Weight:
18 kg
Famotidine Concentration:
10 mg/ml
Standard Dilution Protocol (Varies By Hospital):
Dilute Famotidine with 0.9% NaCl where 8 mg/kg/day = 1 ml/hr and create a 12 hour CRI at that rate
Complete the Math:
(8 mg/kg/day) x (18 kg) = 144 mg/day Famotidine
(144 mg/day) / (24 hr/day) = 6 mg/hr Famotidine
(6 mg/hr) / (10 mg/ml) = 0.60 ml/hr Famotidine
(0.60 ml/hr) x (12 hour CRI) = 7.2 ml Famotidine
(12 ml Total Fluid Volume) - (7.2 ml Famotidine) = 4.8 ml 0.9% NaCl
The Result:
You will need 7.2 ml of Famotidine to mix with 4.8 ml of 0.9% NaCl. This will result in 8 mg/kg/day = 1 ml/hr
