Doctor's Order:
Start a Dobutamine CRI at 6 mcg/kg/min
Patient Weight:
18 kg
Standard Dilution Protocol (Varies By Hospital):
Dilute Dobutamine with 0.9% NaCl where 1 mcg/kg/min = 1 ml/hr and create a 12 hour CRI at that rate
Dobutamine Concentration:
12.5 mg/ml
Convert the Dobutamine from mg to mcg:
(12.5 mg/ml) x (1,000 mcg/mg) = 12,500 mcg/ml concentration of Dobutamine
Complete the Math:
(1 mcg/kg/min) x (18 kg) = 18 mcg/min of Dobutamine
(18 mcg/min) x (60 min/hr) = 1,080 mcg/hr of Dobutamine
(1,080 mcg/hr) / (12,500 mcg/ml) = 0.0864 ml/hr of Dobutamine
(0.0864 ml/hr) x (12 hr) = 1.0368 ml Dobutamine => Rounds to 1 ml Dobutamine
(12 ml Total Fluid Volume) - (1 ml Dobutamine) = 11 ml 0.9% NaCl
The Result:
You will need 1 ml of Dobutamine to mix with 11 ml of 0.9% NaCl. This will result in 1 mcg/kg/min = 1 ml/hr. To run the Dobutamine CRI at 6 mcg/kg/min, increase the fluid rate of your CRI to 6 ml/hr
*Note Per Plumb's as of 10/11/21: "Dobutamine may be physically incompatible with the following agents: aminophylline, bumetanide, calcium chloride, calcium gluconate, diazepam, digoxin, furosemide, heparin (inconsistent results), regular insulin, magnesium sulfate, phenytoin sodium, potassium chloride (at high concentrations [160 mEq/L] only), potassium phosphate, and sodium bicarbonate."
